    The DAC form is required for a customer requesting procurement advice from CPD in respect of a Direct Award Contract.

    For Supplies and Services Division (SSD) DACs clients must use the SSD DAC link below. This will direct you to an online form.

    SSD DAC

    For all other CPD Divisions clients must use the CPD DAC form.
